Your First Visit

Welcome!

During your initial appointment, a consultation will be performed where a diagnosis and treatment options are provided.  If treatment is required, it may be started on the same day.  In some instances, a second appointment is required to complete treatment to help ensure the best outcome possible.

In order to enable us to best serve you, please be prepared to provide the following information at the time of your consultation: 

  • Your referral form and any x-rays of the tooth or teeth if available.
  • A list of medications taken currently or recently.
  • In the event you have medical or dental insurance, please bring any necessary completed forms.  Doing so will help us to process your claim in a timely fashion.

If you have a medical condition that may be of concern prior to treatment (pregnancy, high blood pressure, or diabetes, for example) or if you are on any medication (heart medications, aspirin, for example) or require antibiotics prior to dental treatment, please notify our office prior to your appointment so we can make any necessary accommodations.

 

PLEASE NOTE: For patients under the age of 18 years old, a parent or guardian must accompany them at the consultation visit.

Regarding X-Rays

In order to ensure proper evaluation and diagnosis, x-rays will be taken at our office.  In most instances, a 3-dimensional cone beam x-ray is required to render an accurate diagnosis and to provide treatment options.  It is always our goal to minimize the use of x-rays, and to employ available safety precautions during their use.  If your dentist has recently taken an x-ray, you may request they forward them to our office as well.

Scheduling

Grayslake Endodontics is open Monday – Thursday 8:00 am – 4:30 pm and Fridays 8:00 am – 12:00 pm. We will schedule your appointment at the soonest availability. In the event you are experiencing dental pain or a dental emergency, we will make every effort to see you that same day.

Due to the nature of the treatment we provide (emergency and otherwise), we are occasionally faced with delays.  We appreciate your patience and understanding as we always attempt to minimize your waiting.
